Liability Waiver

RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND INDEMNITY AGREEMENT

In consideration of being permitted to participate in the Jax Legends LLC seminar and related activities (the "Event"), I, the undersigned, hereby agree to the following:

1. Assumption of Risk. I understand and acknowledge that participation in martial arts, wrestling, grappling, MMA instruction, and related physical activities involves inherent risks of injury, including but not limited to sprains, strains, fractures, and other physical harm. I voluntarily assume all such risks.

2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue Jax Legends LLC, its officers, directors, employees, agents, instructors, volunteers, and partners (collectively, "Released Parties") from any and all claims, demands, losses, or liability arising out of or related to my participation in the Event, whether caused by the negligence of the Released Parties or otherwise.

3. Indemnification. I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur as a result of my participation in the Event.

4. Medical Authorization. I consent to receive emergency medical treatment in the event of injury or illness during the Event, and I agree to bear all costs associated with such treatment.

5. Media Release. I grant Jax Legends LLC permission to photograph or record me during the Event and to use such images or recordings for promotional and educational purposes without compensation.

6. Governing Law. This Agreement shall be governed by the laws of the State of Florida. If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
